Job Overview
We are seeking a compassionate and dedicated *Occupational Therapist (OT)* to join our therapy team in a *nursing home / skilled nursing facility*. The OT will work with short-term rehab and long-term care residents to help improve independence with daily activities, safety, and overall quality of life.
Responsibilities
* Conduct occupational therapy evaluations and develop individualized treatment plans
* Provide skilled OT services in accordance with physician orders
* Address ADLs, functional mobility, cognition, upper extremity function, and safety awareness
* Treat residents with orthopedic, neurological, post-surgical, and general weakness diagnoses
* Document evaluations, daily treatment notes, progress notes, and discharge summaries accurately and timely
* Collaborate with nursing staff, physicians, and the interdisciplinary care team
* Educate residents and families on adaptive equipment, energy conservation, and safety strategies
* Ensure compliance with facility policies and state and federal regulations
Qualifications
* Current *Occupational Therapist license* in Arizona
* Graduate of an accredited Occupational Therapy program
* Experience in a SNF or long-term care setting preferred (new graduates encouraged to apply)
* Strong communication, documentation, and organizational skills
* Ability to work independently and as part of a team
