*Job Overview*
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will support patient recovery, comfort, and well-being through skilled nursing interventions. *Responsibilities*
* Deliver comprehensive nursing care by assessing patient needs, monitoring vital signs, and performing physical examinations with precision and compassion.
* Administer medications and injections accurately following physician orders while adhering to HIPAA regulations and safety protocols.
* Assist with medical procedures such as IV infusion, catheterization, wound suturing, and tube feeding to ensure optimal patient outcomes.
* Maintain detailed medical documentation using EMR (Electronic Medical Records) systems like Epic or Cerner, ensuring accuracy and compliance with healthcare standards.
* Support patient assessment and triage in urgent care or emergency settings, prioritizing care based on acuity levels in collaboration with the healthcare team.
* Educate patients and their families on health management strategies, including medication administration, nutrition, dementia care, and chronic disease management.
* Collaborate with interdisciplinary teams to develop discharge plans, coordinate post-acute care, and facilitate smooth transitions across care settings.
*Qualifications*
* Valid state licensure as a Licensed Practical Nurse (LPN) with current certification.
* Proven experience in diverse clinical environments such as hospital medicine, long-term care, hospice, or outpatient clinics.
* Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and diagnostic evaluation techniques.
* Experience working with special populations including seniors, individuals with developmental disabilities, Alzheimer’s disease or dementia care needs.
* Proficiency in using EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation.
* Skilled in patient assessment skills including vital signs measurement, intake evaluation, specimen collection/processing, and basic life support (BLS).
* Ability to perform technical procedures such as venipuncture, phlebotomy, sterile processing techniques, airway management—including ventilator support—and catheter care.
