Arvada Care & Rehabilitation is seeking a Full-Time Speech Therapist to join our fun and hardworking therapy department. We are a 5 star skilled nursing facility with a 5 star inpatient therapy program. Conveniently located in the Denver Metro Area, Arvada Care & Rehab is a 52 bed skilled and long term care facility providing top tier care to the community. Duties and Responsibilities:
* All duties and responsibilities shall be performed as set forth in our established policies and procedures.
* Adheres to and assures compliance with Code of Conduct, facility policies and procedures and all applicable rules, regulations and standards as promulgated by Federal, State, and accrediting agencies or regulating bodies. This includes, but is not limited to, Department of Health, Centers of Medicare and Medicaid Services, and other applicable regulatory agencies.
* Completion of required compliance training and processes
* Effectively screens/evaluates patients and/ with communication, cognitive or swallowing disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
* Utilizes standardized assessments and evidence-based practice to support clinical interventions.
* Provides rehabilitative, skilled and medically necessary treatment intervention to patients with communication, cognitive and/or swallowing disorders per regulatory and clinical practice requirements.
* Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
* Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
* Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the speech disorders.
* Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
* Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
* Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy.
* Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
* Participation in required meetings including but not limited to: patient care conferences, utilization review meetings, rehabilitation meetings, etc.
* Participate in facility and company required trainings, in-services and conferences.
* Able to obtain any necessary DME or supplies per regulatory and clinical practice requirements.
* Adherence to State Required practice acts, code of conduct, ethics during professional practice as a Speech-Language Pathologist.
* Adherence to Company required policy and procedures and code of conduct.
* Active State Licensure
* Performs other duties as may be assigned.

KNOWLEDGE, SKILLS & ABILITIES:
Educational Requirements:
* Must be a graduate of an accredited program in Speech-Language Pathology
Specific Requirements:
* Must be licensed with applicable state
* One year’s experience in long-term care or related setting preferred
WORKING CONDITIONS:
* Subject to injury from falls, burns from equipment, odors and is exposed to dust, disinfectants, hazardous chemicals, tobacco smoke, and other air contaminants.
* Subject to hostile and emotionally upset residents, family members, personnel, visitors, or others.
* Subject to exposure to infectious waste, diseases, and conditions, including Tuberculosis and the AIDS and Hepatitis B viruses.
Physical Mental and Sensory Requirements (With or Without the Aid of Mechanical Devices)
* Move intermittently throughout the work day.
* Speak and write the English language in an understandable manner.
* Cope with the mental and emotional stress of the position.
* Possess sight/hearing senses,
* or use prosthetics that will enable these senses to function adequately so that the requirements of the position can be fully met.
* Meet the general health requirements set forth by the policies of this facility which include a medical and physical examination.
* Assist in the evacuation of residents during emergency situations.
Physical Requirements
Activity Strength Requirements SCALE:
* S=Sedentary Work- Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull, or otherwise move objects, including the human body. Sedentary work involves sitting most of the time,but may involve walking or standing for brief periods of time.
* L=Light Work- Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. Light work usually requires walking or standing to a significant degree.
* M=Medium Work- Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
* H=Heavy Work- Exerting up to 100 pounds of force occasionally, and/or excess of 50 pounds of force frequently, and/or excess of 20 pounds of force constantly to move objects.
* V=Very Heavy Work- Exerting in excess of 100 pounds of force occasionally, and/or excess of 50 pounds of force frequently, and/or excess of 20 pounds of force.
