Zenith Health Contractors LLC is a clinician-led home health contracting company serving California’s Central Valley.
We partner with established home health agencies and work with experienced Physical Therapists who want flexibility, efficiency, and strong per-visit compensation — without the limitations of traditional staff roles.
This opportunity is built for PTs who are comfortable in home health and looking for a flexible opportunity that maximizes income.
COMPENSATION (REAL NUMBERS)
* Per-Visit Rate: $100–$180 per visit, depending on experience and visit type
* Pay Structure: Pay-per-visit (1099 Independent Contractor)
* Caseload: Clinician-directed. No quotas. See as many patients as you choose.
* Earning Potential: High-volume clinicians routinely exceed $10,000–$12,000+ per month

WHAT YOU’LL BE DOING
* Perform Start of Care, Resumption of Care, Evaluations, Reassessments, and Discharges
* Deliver skilled PT services in the home health setting
* Address pain, strength, balance, gait, mobility, and functional impairments
* Develop and progress individualized plans of care
* Educate patients and caregivers on HEPs, safety, and fall prevention
* Communicate effectively with PTAs and agency teams
* Complete timely, accurate documentation via EMR
WHAT WE’RE LOOKING FOR (REQUIRED)
* Active California Physical Therapist license
* Minimum 1+ year of home health PT experience
* Comfortable managing full episodes of care independently
* Strong clinical reasoning and time-management skills
* Proficiency with digital documentation
* EMR experience with Kinnser and/or Devero preferred
* Reliable transportation and valid driver’s license
* Current CPR certification
* Professional, accountable, and self-directed
COVERAGE AREA
Atwater | Merced | Surrounding Communities (≈30-mile radius)
Geographic efficiency prioritized.
