*Summary*
'We are seeking a dedicated Occupational Therapist to join our compassionate team. In this role, you will help patients regain independence and improve their quality of life through personalized therapy plans. Your work will directly support our mission to provide exceptional home health care and make a positive difference in our community.'
*Responsibilities*
' *Assess patients' needs and develop individualized treatment plans.*
* Assist patients with activities of daily living (ADLs) to promote independence.
* Coordinate discharge planning and collaborate with healthcare teams.
* Provide therapy services for patients with various conditions including geriatrics, pediatrics, and disabilities.
* Educate patients and families on therapeutic exercises and safety.
* Maintain accurate documentation of patient progress and care plans.
* Support case management efforts to ensure comprehensive patient care.
* Adhere to all healthcare regulations and safety protocols.'
' *Valid occupational therapist license in the state of Texas.*
* Proven experience in home health or outpatient settings.
* Strong knowledge of ADLs, discharge planning, and case management.
* Excellent communication skills for working with diverse patient populations.
* Ability to develop effective treatment plans tailored to individual needs.
* Compassionate approach aligned with our company values.
* Preferred: ICU or acute care experience; pediatrics or geriatrics background; familiarity with medical terminology and physiology.'
