Your Purpose in This Role
As a
Transitional Care Liaison , you’ll be the bridge between Touchstone Communities and healthcare providers, driving sales through
value-based selling
and building strong relationships. You’ll represent our skilled nursing communities and showcase clinical outcomes that matter.
What You’ll Do
Develop and execute a
Regional Sales Plan
Act as a liaison with hospitals, SNFs, and healthcare providers
Communicate effectively with communities and regional teams
Manage CRM reporting in
Point Click Care
Ensure compliance with HIPAA, infection control, and company policies
What We’re Looking For
2+ years of healthcare sales/business development/marketing industry experience
Proficiency in
Microsoft Office ; CRM experience is a plus
Ability to work
independently and collaboratively
Experience marketing
Long-Term Care/Post-Acute/Hospital or SNFs
in the greater Houston, TX region is required.
