We are recruiting a 
fulltime Care Coordinator
 – Office hours between 8:30am and 5pm weekdays.

Duties include:

  • Coordinate scheduling and management of Client and Employee files using our web-based software
  • Engage with our frontline workers to match our clients' needs with their availability
  • Gather necessary information to initiate client services/new intakes
  • Provide on-going feedback to families regarding service delivery and continuous assessment of patient satisfaction using email or video conferencing
  • Provide on-going support to staff regarding care issues that surface during service delivery
  • Inform the Head of Operations of staffing issues (training, skills, performance, etc) that require follow-up
  • Manage services between Equinoxe services and local community resources
  • Update client and employee files daily (as required)
  • Complete appropriate paperwork to comply with Equinoxe policy and procedures
  • Compliance with Equinoxe LifeCare Solutions policies and procedures regarding confidentiality, dress code, ethical standards, and patient care

The ideal candidate for this role should possess a strong commitment to helping others and be able to work collaboratively with our team members. Candidate must have strong language skills (English and French).

  • Candidate must be "tech savvy" and be proficient in Microsoft Office.
  • Educational level: DEC or higher
  • Years of experience related to the job offer: 3-5 years relevant work experience

Description of qualifications:

- The successful candidate will be responsible, calm/patient, diplomatic and have a good, professional telephone manner and excellent customer service skills

- Attention to details and follow-up and ability to learn and use our care management software program are essential

  • Prior experience in the healthcare sector is an asset

  • Job Type: Permanent
  • 40 hours per week
  • To begin as soon as possible
  • In person with possibility of one remote workday per week
