The Foot Care Nurse is a vital part of Dufferin Area Family Health Team (DAFHT) and collaborate with our physicians and other interdisciplinary providers to achieve the vision and mission of the organization.
DUTIES AND RESPONSIBILITIES:
1. Assessment:
· Conduct foot health assessments and collaboratively identify priorities for treatment
2. Implementation and Evaluation:
· Perform routine nail care and foot care duties, such as cutting, trimming, filing and removing ingrown nails, calluses, corns and fungus and conducting diabetic foot care assessments
· Develop appropriate treatment plans and provide appropriate foot care education while adapting teachings to the client's identified needs
· Document in clients chart according to Standards of Practice as determined by the DAFHT
· Refer clients to other interdisciplinary resources and arrange follow-up as necessary
· Evaluate the client's comprehension and compliance to the foot care plan by reviewing clinical and learning outcomes
3. Team Development:
· Promote awareness of the DAFHT's services and programs
· Participate in staff and team meetings as appropriate
· Participate in the development, planning and evaluation of treatment, education, counselling and health promotion activities of the FHT as required
4. Professional Development:
· Maintain and develop professionalcompetence through ongoing learning and development
· Stay current and aware of opportunities to implement evidenced-based methods of client assessment, treatment and programming
5. Communication:
· Communicate effectively with team members, clients, families, peers, other health care professionals and community partners to provide cohesive and seamless services to the community
· Collect and report on data and relevant information as required
6. Health and Safety:
· Ensure that all foot care equipment is cleaned and sterilized according to Standards of Practice for Chiropodists and Podiatrists - Infection Prevention and Control as set out by the College of Chiropodists of Ontario
· Employ appropriate Personal Protection Equipment and Barriers
· Perform requisite quality control activities on all chiropody equipment as indicated
7. Related Duties:
· Always maintain confidentiality of team and client information
· Ensure efficient and effective operation of foot care services by ordering supplies and updating inventory as required
EDUCATION/EXPERIENCE:
· Registered RN or RPN in good standing with the College of Nurses of Ontario (CNO)
· Certification from an accredited Advanced Foot Care course
· Three years foot care experience working in a community-based setting with experience working in a Family Health Team a definite asset
· Current Basic CPR certification
· Current Ontario Drivers' License
SKILLS/ABILITIES:
· Excellent oral and written skills
· Ability to work effectively with all members of the health care team
· Knowledge and proficiency in current, evidence-based methods and practices of primary care delivery
· Ability to develop strategies that successfully meet the needs of a diverse patient, family and provider population
· Proficiency in the use of computer hardware and software
· Ability to prioritize, manage time effectively and be flexible in a very active work environment
