We are seeking a licensed *Physical Therapist (PT)* to join our home health team. The ideal candidate is independent, flexible, and passionate about helping patients achieve their mobility and rehabilitation goals in a home setting.
*Responsibilities*
* Perform in-home physical therapy evaluations and develop individualized care plans
* Implement treatment plans and provide patient education and training
* Monitor patient progress and adjust plans as needed
* Communicate with physicians, nurses, and other healthcare professionals
* Document visits and maintain accurate records in accordance with agency and regulatory standards
* Travel to patients’ homes within the assigned geographic area
*Qualifications*
* Current *Physical Therapist (PT)* license in the state of Michigan
* Minimum 1 year of physical therapy experience (home health experience preferred)
* Valid driver’s license and reliable transportation
* CPR certification
* Strong communication and time management skills
* Experience with HCHB (preferred)
