The Inpatient Master of Social Work (MSW) Care Manager provides professional, comprehensive patient centric care management services for at risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. We are hiring for a Care Manager - MSW position at Providence Alaska Medical Center in Anchorage, AK!
