*Responsibilities*
* Assess patients' needs and develop individualized treatment plans based on their specific conditions and goals.
* Provide direct patient care, including assistance with activities of daily living (ADLs) and therapeutic exercises.
* Collaborate with multidisciplinary teams to ensure comprehensive care and effective discharge planning.
* Document patient progress and modify care plans as necessary, focusing on improving functional abilities.
* Educate patients and families about treatment options and strategies for continued care.
