Join our dedicated healthcare team as a Home Health Physical Therapist and make a meaningful difference in patients' lives by delivering expert physiotherapy care in the comfort of their homes. This dynamic role offers the opportunity to work independently while collaborating with a multidisciplinary team to promote recovery, mobility, and independence. You will assess patient needs, develop personalized care plans, and implement therapeutic interventions that foster healing and improve quality of life across diverse populations, including pediatrics, geriatrics, and orthopedics.
*Duties*
* Conduct comprehensive evaluations of patients’ physical condition, including mobility, strength, balance, and functional abilities.
* Develop and implement tailored care plans that align with patient goals and medical directives.
* Administer physiotherapy treatments such as manual therapy, trigger point therapy, and therapeutic exercises to enhance recovery.
* Assist with discharge planning by coordinating follow-up care and educating patients and caregivers on home exercise programs.
* Monitor patient progress regularly, adjusting treatment strategies to optimize outcomes.
* Document all assessments, treatments, and progress notes accurately within electronic medical record (EMR) systems.
* Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure comprehensive patient care across inpatient, outpatient, hospital, home health, and community settings.
*Requirements*
* Valid license or registration as a licensed Physical Therapist in the relevant jurisdiction.
* Proven experience in home health care or community-based physiotherapy services.
* Strong knowledge of anatomy, physiology, medical terminology, and care plan development.
* Experience working with diverse populations including pediatrics, geriatrics, orthopedics, and patients recovering from acute care or inpatient stays.
* Familiarity with EMR systems for documentation and patient management.
* Ability to perform discharge planning effectively while coordinating with multidisciplinary teams.
* Excellent patient care skills with a compassionate approach to therapy delivery.
* Knowledge of outpatient clinic procedures and hospital protocols is preferred.

Pay: From $45.00 per hour
