Position Overview

The Licensed Practical Nurse (LPN) at The Social at Savannah plays a key role in delivering compassionate, resident-centered care. The LPN supports residents' health and wellness by providing clinical oversight, administering medications, assisting with care plans, and ensuring a safe and supportive environment. This position is essential in upholding Thrive's mission to create a community where older adults can thrive with purpose, connection, and joy.

Key Responsibilities

  • Provide direct nursing care to residents in accordance with physician orders, state regulations, and company policies.
  • Administer medications, treatments, and injections safely and accurately.
  • Monitor residents' health status, document observations, and communicate changes to the Wellness Director and/or physician.
  • Assist in developing, implementing, and evaluating individualized care plans.
  • Supervise and support Caregivers and Med Techs, ensuring proper training and adherence to care standards.
  • Respond promptly and professionally to resident emergencies or urgent care needs.
  • Collaborate with residents, families, and the care team to promote wellness and engagement.
  • Maintain accurate, detailed, and timely medical records and documentation.
  • Ensure compliance with state and federal regulations, as well as Thrive Senior Living policies and procedures.
  • Participate in resident and family education regarding health and wellness.
  • Foster a positive, respectful, and supportive environment for residents and team members.

Qualifications

  • Current and valid LPN license in the state of employment.
  • Prior experience in senior living, long-term care, or healthcare setting strongly preferred.
  • Excellent clinical, organizational, and communication skills.
  • Ability to lead and mentor team members with patience and professionalism.
  • Compassionate approach with a genuine passion for serving older adults.
  • Flexibility to work varying shifts, including evenings, weekends, and holidays.
  • Current CPR certification preferred.
