At *Salem Transitional Care of Cascadia Healthcare*, we’re dedicated to providing exceptional skilled nursing and rehabilitation services that empower our residents to recover, thrive, and live well. We’re currently looking for a *Physical Therapist (PT)* to join our dynamic therapy team. This is a great role for a clinician who enjoys helping adults improve strength, mobility, and overall function in a supportive, resident-focused environment.
*What You’ll Do*
* Complete PT evaluations and create individualized treatment plans
* Provide hands-on therapy to improve balance, gait, endurance, and functional mobility
* Modify treatment plans based on resident progress and clinical judgment
* Educate residents and families on exercises, safety, and home strategies
* Document evaluations, treatment notes, and progress updates in a timely manner
* Collaborate with PTAs and partner with the interdisciplinary team
*What You Bring*
* Degree in Physical Therapy from an accredited program
* Active Oregon PT license (or eligibility)
* CPR certification (or ability to obtain)
* SNF, post-acute, or geriatric experience preferred—but not required
* Strong communication, teamwork, and clinical reasoning skills
