*Overview*
We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in ensuring the safe and effective use of medications for our patients. This position requires a strong understanding of clinical pharmacy practices, medication management, and patient care. The Pharmacist will collaborate with healthcare professionals to provide optimal pharmaceutical care, educate patients on medication usage, and contribute to the overall health and well-being of our community.
*Responsibilities*
* Review and interpret physician orders to ensure safe and effective medication therapy.
* Prepare and dispense medications accurately, including IV infusions and other complex formulations.
* Provide immunizations and educate patients on vaccine protocols.
* Conduct medication therapy management and pain management consultations.
* Monitor patient outcomes and adjust medication regimens as necessary.
* Collaborate with healthcare teams to develop treatment plans based on clinical assessments.
* Maintain comprehensive knowledge of anatomy, pharmacology, and critical care practices.
* Participate in pharmacy residency programs or precept pharmacy students as needed.
* Ensure compliance with all regulatory requirements and maintain accurate records.
*Experience*
* Doctor of Pharmacy (Pharm.D.) degree from an accredited institution is required.
* Completion of a pharmacy residency is preferred but not mandatory.
* Experience in clinical pharmacy settings or critical care environments is advantageous.
* Proficiency in IV insertion techniques and medication administration protocols is highly desirable.
* Strong communication skills with the ability to educate patients effectively about their medications.
* A commitment to continuous professional development in the field of pharmacy practice.
