Therapy Advantage is looking for a physical therapist for our home health therapy company in the greater Toledo area. Home Health and SOC OASIS experience is a plus but not necessary. We offer flexibility to create your own schedule.
*Summary:* Plans and administers medically prescribed physical therapy treatment for patients suffering from injuries, or muscle, nerve, joint and bone diseases, to restore function, relieve pain, and prevent disability by performing the following duties.
*Essential Duties and Responsibilities*: Include the following. Other duties may be assigned.
* Reviews Physician's referral (prescription), patient's condition and medical records to determine physical therapy treatment required. Contacts referring physician after evaluation complete to receive verbal order to continue POC, to ensure services are covered under a physician's order.
* Tests and measures patient's strength, motor development, sensory perception, functional capacity, and respiratory and circulatory efficiency, and records findings to develop or revise treatment programs.
* Plans and prepares written treatment program (poc) based on evaluation of patient data.
* Administers manual exercises to improve and maintain function.
* Instructs, motivates, and assists patient to perform various physical activities and in use of assistant and supportive devices such as walkers, canes, and prostheses.
* Administers treatments involving application of physical agents.
* Evaluates the effectiveness of treatment (ie: Medicare 13/19/30 reassessments) at various stages and adjusts treatments to achieve maximum benefit.
* Documents skilled treatment every visit, patient response, and progress toward goals.
* Instructs patient and family in treatment procedures to be continued in the home.
* Evaluate for prosthetic and orthotic devices and recommend modification to Orthotist.
* Confers with Physician and other practitioners, specifically nurse case managers to obtain additional patient information, suggests revisions in treatment program, and integrates physical therapy treatment with other aspects of patient's health care.
* Coordinates with other disciplines for reassessments to meet Medicare requirements. Coordinates with HH agencies regularly, particularly to get approval for extensions, recerts and discharges.
* Orients, instructs, and directs work activities of assistants, aides, and students.
*Supervisory Responsibilities:* Directly supervises 3-6 Assistants in the Physical Therapy team.
Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.
Directing work, appraising performance, resolving patient issues, checking for completion of daily notes and assuring goals, vitals, skilled documentation are addressed as part of the supervisory role. Maintains availability via phone for supervision; Performs reassessments on time to avoid missed treatments.
Pay: From $70.00 per hour
