Position Summary:
The Practice Facilitator is a champion for change working collaboratively with family physicians, clinic managers, and Primary Care Network (PCN) staff. Must exhibit readiness in quality improvement to plan, implement, and evaluate clinical initiatives that target patient-centered care within the Patient's Medical Home.
The Practice Facilitator will work as part of the Quality Improvement team to support member clinics through identifying issues and working out solutions aligning with clinic processes and values.
Key Responsibilities:
Clinic Support
- Establish professional relationships with clinic teams, physicians, and clinic managers.
- Work with physician leads to motivate and inspire clinic teams to action change.
- Understand clinic team goals for improvement and connect their ideas to the Patient's Medical Home model.
- Champion and support clinical staff in facilitating process improvements.
- Introduce provincial quality improvement opportunities to support clinic teams.
Quality Improvement / Facilitation
- Develop, implement, and evaluate practice improvement strategies with member physicians, clinical teams, and centralized PCN clinics to improve the quality and delivery of care.
- In collaboration with the Quality Improvement team, develop tools and protocols where necessary for quality improvement initiatives.
- Implement and monitor practice change methodologies appropriately using clinical practice and outcome measures.
- In collaboration with the Quality Improvement team, build capacity for continuous quality improvement with clinics and interdisciplinary teams.
Relationship Building and Collaboration
- Develop and maintain collaborative relationships between clinic teams, physician leads, and PCN staff.
- Support and encourage teams to share their improvements and lessons learned with other practices within the PCN.
- Grow and sustain the Patient's Medical Home through change management initiatives.
- Leverage relationship with Practice Facilitator to collaborate on improvement initiatives.
Education:
- Degree or Diploma in a health-related field, preferably nursing or equivalent years of experience working in healthcare
Qualifications:
- 5+ years in primary healthcare or working directly with clinic teams
- Comprehensive understanding of Primary Care and the Patient's Medical Home
- Experience in group facilitation and change management
- Experience and knowledge of clinical workflow
- Ability to foster a collaborative environment and manage conflict with diplomacy and tact
- Proficiency in written and verbal communication
- Experience in Primary Care is highly preferred.
- Proficiency in one or more major EMRs used in Alberta (Med Access, Healthquest, Accuro)
- Proficiency in Microsoft Office Suite
- Valid Driver's License and access to a reliable vehicle for travel
- Strong interpersonal skills and verbal and written communication
