*Job Responsibilities*
Determine plan of care and rehabilitation goals based on functional deficits and provide prescribed and necessary treatments in accordance with the treatment plan to improve and/or restore the patient to safe and independent functioning
Completes comprehensive assessments incorporating the OASIS data set, as required by Medicare, utilizing OASIS information to assist in the development of the plan of care
Maintains confidentiality of OASIS data set, patient medical records, all patient protected health information, and BNI material and business
Evaluates patients, families, home environment for appropriateness of admission and verifies the patient’s eligibility for the Medicare home care benefit, including homebound status, at the time of initial assessment visit and comprehensive assessment
Contacts physician following evaluation to discuss patient status and obtain orders for ongoing physical therapy visits, as indicated
Assists the physician in evaluation the patient’s level of function, and helps develop, and revise as necessary, the plan of care, in collaboration with the home health team
Identifies goals, interventions and expected outcomes, and involves the patient and/or caregivers in the plan of care and any changes in plan depending on ability of patient/caregiver
Establishes, develops and revises, as necessary the plan of treatment in collaboration with the physician and the home health team, to fully address the patient’s overall needs based on evaluation of deficits and pertinent to medical history
Evaluates effectiveness of program outcomes of care, and patient progress
Provides Physical Therapy services as prescribed by the physician, which can safely be provided in the home
Assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional abilities tests, as indicated by subjective complaints and objective findings of the evaluation
Assists the physician in evaluating disability, injury or disease process of individual patients, and in determining the patient’s level of function
Designs, establishes and implements rehabilitation programs, based on patient assessment, identified patient goals/outcomes, and needed interventions, in accordance with physician orders and APTA best practice patterns
Consults with physician when necessary and documents contents of conversation. Creates change orders to reflect changes in Plan of Care orders
Follows the physician’s Plan of Care and communicates changes to all appropriate disciplines/caregivers involved in the patient’s care
Communicates with the physician at least every sixty (60) days, when patient’s condition changes or there is a deviation in the plan of care, regarding the patient’s status, obtaining additional orders if necessary
Prepares records and submits clinical and progress notes in a timely manner following visit, or communication
Provides patient/caregiver education with a focus on goal and outcome achievement as established in the plan of care, documenting patient response to teaching in clinical notes
Teaches and supervises staff, including home health aides when appropriate, patients and caregiver regarding rehabilitation program, therapeutic activities to restore safe physical function, and use of physical therapy assistive devices, as appropriate to patient’s level of need
Evaluates the patient for appropriate orthotic device, prosthetic device and/or assistive devices, communicates DME needs with Physician and designs and fits or orders equipment, as indicated
Assumes responsibility for equipment, cleaning, and maintenance, as appropriate
Develops and implements ongoing discharge plans starting at admission, including coordination of care with other disciplines and evaluation of patient/caregiver understanding and response
Supervises Physical Therapist Assistants and Home Health Aides, as required. Remains readily available and accessible at all times by two way communication for consultation during the delivery of care
Attends mandatory staff meetings and participates in patient case conferences/coordination of care communication
Submits necessary and required documentation on company approved forms in a timely manner
Participates in staff orientation and clinical continuing education programs as required/requested
Participates in Performance Improvement activities and identifies areas for improvement
Participates in clinical record review as directed by the Clinical Supervisor
Acts as a consultant to Agency staff and Home Health Team, as necessary, to effectively coordinate patient’s rehabilitation treatment regimen and overall care
Complies with Departmental and Company Policies and Procedures
Maintain standards of practice and documentation that are consistent with Federal, State, Local, Medicare, and accreditation regulations and standards
Demonstrate flexibility while performing job duties to ensure the highest quality of service is provided
Utilize critical thinking skills while performing job duties; analyze work process and make suggestions for improvement
Maintain a thorough understanding and consistently adhere to all applicable department and company policies and procedures
Conduct job duties in a way which will reduce and avoid potential safety hazards
Demonstrate initiative and drive to promote personal and company success
Perform other duties as assigned by your supervisor or management team member
