Star Home Health is committed to delivering compassionate, high-quality home health services to patients in the comfort of their homes. We are seeking a dedicated and skilled *Physical Therapist (PT)* to join our growing team of clinicians.
*Position Summary:*
The Physical Therapist provides skilled physical therapy services to home health patients, focusing on improving mobility, strength, safety, and independence. This role involves comprehensive evaluations, individualized treatment planning, and ongoing therapeutic interventions.
*Key Responsibilities:*
* Perform initial evaluations and develop personalized physical therapy care plans
* Provide therapeutic exercises, gait training, balance training, and mobility improvement interventions
* Educate patients and caregivers on safe mobility, home exercise programs, and equipment use
* Complete timely and accurate clinical documentation, including evaluations, progress notes, and discharge summaries
* Collaborate with interdisciplinary team members, including nurses, OTs, STs, and physicians
* Reassess and adjust treatment plans based on patient progress and clinical needs
* Participate in case conferences and support coordinated patient care
* Ensure compliance with Medicare, state regulations, and agency policies
*Qualifications:*
* *Active Arizona Physical Therapist (PT) License – Required*
* Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy
* Home health experience preferred but not required
* Strong clinical evaluation and documentation skills
* Excellent communication and time-management abilities
* Ability to work independently in the field
* Reliable transportation and valid driver’s license
Job Types: Part-time, PRN, Contract, Per diem
