Since its founding as a teen drop-in center in 1972, Community Solutions (CS) has grown in both depth and breadth of services in response to the growing and changing needs in the community. As one of the most comprehensive nonprofit human services agencies in Santa Clara County and the surrounding areas, CS provides prevention, intervention, treatment, and residential services to more than 6,400 clients annually, helping them overcome the challenges posed by mental health issues, substance abuse, trauma, severe family dysfunction, sexual and domestic violence, and human trafficking.
Under the direction of the Program Manager, the Occupational Therapist II is responsible for utilizing clinical knowledge and judgment to provide developmental screenings and focused interventions to individuals to support their mental health recovery. Provide consultation and program support to staff as requested. Services are provided throughout Santa Clara County.

*Education and Experience:*
Required:
* Requires a minimum of a master's degree in occupational therapy from an accredited program.
* At least one year of experience as an Occupational Therapist, preferably in a mental health or school setting.
Preferred:
* Bilingual and bicultural candidates are strongly encouraged to apply.
Required:
* Valid California Occupational Therapy License issued by the California Board of Occupational Therapy (CBOT).
* Demonstrated skill in assessing and reassessing a person’s functional capabilities.
* Ability to identify and establish individual treatment plan goals for occupational therapy.
* Knowledge, treatment, rehabilitation, and assessment techniques for multiple age groups and populations.
* Knowledge of developmental concerns and behavior including foundations of mental health; sensory and developmental concerns as well as effective strategies; advocacy and outreach strategies; working with individuals/families who have diverse economic, social, racial and cultural backgrounds.
Preferred:
* Knowledge and/or certification in evidence-based practices and outcome driven care.
*What You'll Be Doing in the role of Occupational Therapist II: *The following are examples of essential job duties for this position. The omission of specific statements of the duties does not exclude them from the classification if the work is similar, related, or a logical assignment for this classification. Other duties may be required and assigned to support the program and Agency.
* Utilizes clinical best practices to conduct screening, intake and assessments to determine the client’s needs and strengths to support development of treatment goals.
* Community based work to include home visits, work/employer visits, and school-based services.
* Performs integrated occupational therapy-based interventions with clients and families as needed.
* Employs evidenced-based practices and the wellness & recovery model as appropriate to assist the client and identify issues of dual diagnosis, activities of daily living, employment, mental illness, physical illness, relationships, and housing which are impacting their mental health and successful recovery.
* Completes detailed documentation in accordance with agency, program and regulatory requirements.
* Maintains all charts within Agency and Medi-Cal regulations.
* Teaches life skills, assists individuals and families with educations and support activities, accesses appropriate community services and rehabilitation counseling.
* Develops and maintains collaborative relationships with appropriate community organizations and system providers.
* Employees must have the ability to operate their personal motor vehicle to participate in client visits, provide daily transportation of clients and/or other assigned Agency business.
* Respond to phone calls and emails within 24 hours on business days. Resolve outstanding issues collaboratively using strength-based solutions focused approach.
* Attends and participates in Agency meetings and contributes to continuous quality improvement activities to enhance team and program development.
* Maintains excellent customer service skills within internal and external customers.
* Successfully completes all training requirements for this position to ensure agency and contract compliance.
* Participates in on-call and after hour's rotation, as assigned. A flexible schedule may be required to work some evenings and weekends.
* Performs first aid as needed based on training and certification.

The annual base salary range for this role is $100,376.64 - $106,520.44. Grade level and salary ranges are determined through interviews and review of education, experience, knowledge, skills, abilities of the applicant, equity with other team members, and alignment with the market data. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. *Work Environment and Location:* This position is located within commutable distance to our Gilroy headquarters or San Jose office and is open to a hybrid work schedule, with occasional needs to be able to travel into our offices and work in the evenings or weekends.
*Special Requirements:*
* Must be able to meet and receive criminal records clearance as required by specific programs, licensing regulations and Community Solutions practices.
* License: Possess a valid California driver’s license, current car insurance and a good driving record as documented by a DMV report and consistent with the Agency’s liability insurance requirements.
* Requires an automobile to perform routine job duties.
