We are seeking a detail-oriented and experienced Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team as an MDS Coordinator in a Skilled Nursing Facility (SNF). In this role, you will oversee the completion of Minimum Data Set (MDS) assessments in accordance with federal and state guidelines, ensure regulatory compliance, and collaborate with interdisciplinary teams to deliver high-quality, patient-centered care. The ideal candidate has a strong background in case management, excellent clinical assessment skills, and is proficient in electronic health record systems such as PointClickCare.
*Key Responsibilities:*
* Coordinate and manage the timely and accurate completion of MDS assessments for residents in accordance with CMS regulations and facility policies.
* Ensure compliance with all regulatory and reimbursement requirements, including OBRA and PPS guidelines.
* Collaborate with nursing, therapy, social services, dietary, and other disciplines to develop and update individualized care plans based on comprehensive resident assessments.
* Evaluate residents’ physical, emotional, cognitive, and psychosocial needs through regular assessments and data collection.
* Participate in care plan meetings and quality assurance reviews to ensure optimal outcomes for residents.
* Maintain accurate and thorough documentation in the electronic health record system (Epic preferred).
* Communicate effectively with managed care organizations to ensure appropriate reimbursement and accurate case mix indexing.
* Provide case management support, including coordinating services and discharge planning to support continuity of care.
* Educate staff on MDS-related processes and support quality improvement initiatives.
* Serve as a resource for staff and leadership regarding MDS regulations and documentation best practices.
*Qualifications:*
* *Licensure:* Current and valid LPN or RN license in FL.
* *Experience:* Minimum of 2 years of experience in a skilled nursing facility or similar healthcare setting, with a focus on MDS coordination or case management.
* In-depth knowledge of MDS 3.0 assessment processes, PDPM, and relevant CMS regulations.
* Strong clinical judgment and assessment skills.
* Excellent communication, collaboration, and interpersonal skills.
* Highly organized with the ability to prioritize and manage multiple tasks effectively.
* Proficiency with electronic health records (Epic or similar platforms preferred).
