Skilled Nursing Facility LPN

WNY’s Preeminent Life Care Community is seeking a compassionate and highly motivated LPN to join our team on our fifty (50) bed person-centered care CARF accredited Skilled Nursing unit in Williamsville, NY. Open positions:
Part Time (2 days/week): 7am - 3pm
Part Time Every Other Weekend (1 day/week): 7am - 3pm
Regular Part Time (3 days/week): 3pm - 11pm with a $2,500 retention bonus
Part Time (2 days/week): 11pm - 7am

Job purpose:

The primary purpose of this job position is to provide supervision of procedures that maximize the resident's existing abilities, emphasize independence instead of dependence, and minimize the effects of aging with an attitude of realistic optimism under the supervision of an Administrator.

Job tasks include:
Participate in the development and implementation of care plans, including quarterly reviews, as they relate to care.
Prepare, administer and document medications as per procedure.
* Demonstrate to residents and staff personnel, as necessary, the procedures involved in the resident care.
* Encourage residents to perform or participate in ADL functions and range of motion exercises. Assists residents with range of motion as necessary.
* Collect data and report resident concerns to RN/MD or interdisciplinary team as applicable for follow-through. Assure directions and follow-through on unit and completion of documentation.
* Conduct regular walking rounds of nursing unit.

LPN Qualifications:
Must possess a current, unencumbered, active license to practice as an LPN in New York State or permit.
Training in rehabilitative and restorative nursing practices is preferred.
* Must pass a medical evaluation, background check, and drug test.
* Must have reliable transportation.
