*Statement of Purpose: *The Pharmacies are located in Tampa and St Petersburg. The Pharmacist will coordinate and assist in managing and participating in pharmacy services, including mostly dispensing of medications, quality assurance and clinical monitoring activities. *The Pharmacist Float will be based in St. Petersburg and float to* *our Tampa pharmacy as needed.*
Our health centers provide an array of health care services including but not limited to primary care (adult & pediatric), HIV treatment, HIV Prevention treatment (PrEP/nPEP), and hormone replacement therapies. The pharmacy serves a diverse patient population in a community health care setting.
* Primary Tasks/Responsibilities:
* Ability to float between St. Petersburg and Tampa.
* Ensure the accurate delivery of pharmaceutical care
* Receive physician prescription orders and evaluate the appropriateness of these orders for dosage, drug choice, potential drug interactions, route of administration and stability.
* Reviews, interprets, clarifies, enters, and verifies patient’s information and medication orders for processing.
* Ensure that the patient chart is complete for allergies, height, weight, current and past medical history, clinical laboratory results and concurrent medications (including over-the-counter or home remedy medications) and documentation of communication with providers/nurses/ patients
* Provide patient counseling, education, and monitoring
* Ensure safety of patients and personnel by exercising good pharmacy practice in the process of drug therapy instructions to the patient
* Determine remaining doses on hand to manage inventory and avoid waste
* Assess responses to therapy, patient compliance, and ongoing review of drug regimens; report and manage adverse events.
* Participate in quality improvement initiative, research projects and provider education.
* Assist in the maintenance and upkeep of all necessary paperwork required by state and federal agencies.
* Assist in the training and supervision of Pharmacy Technicians, Pharmacy Interns and other pharmacy personnel
* Assist in providing in-service programs to nursing, medical and other health center personnel on any aspect of drug therapy
* Reviews and assesses medication orders to identify, prevent, and resolve potential and actual drug related problems.
* Advises prescriber of medication related issues and recommends formulary alternatives when appropriate.
* Maintain current marketplace knowledge regarding trends, new drug developments, updated standards of clinical care, and business strategy topics aligned to organizational growth strategy
* Member of the Performance Improvement (PI) Committee
* Other duties as assigned.
* Education/Professional:
* Bachelors of Science Degree in Pharmacy or Doctor of Pharmacy Degree from an American Association of Colleges of Pharmacy (AACP) accredited university.
* Licensed as a Pharmacist with the Florida Department of Health, Board of Pharmacy or eligible for reciprocation in current state within 30 days of hire.
* Minimum of 2 years of experience in clinical practice or pharmacy. _Experience in Specialty Pharmacy_ _highly preferred_.
* Current Basic Life Support (BLS) certification
*Knowledge, Skills and Competencies Required:*
* Within scope of job, requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision.
* Demonstrates knowledge and/or experience with 340B pharmacy program, documentation, and compliance.
* Demonstrates strong organizational, interpersonal, and communication skills
* Knowledge of use of pharmacy software programs, electronic medical records systems and Microsoft office products.
* Ability to apply principals of health promotion and disease prevention.
* Ability to efficiently and accurately manage multiple tasks at one time.
* In depth current knowledge in area of specialties including HIV treatment, HIV Prevention treatment (PrEP/nPEP), Hormone Replacement Therapies, and others related care provided at METRO’s health centers.
*Requirements:*
* Must possess and maintain valid Florida driver’s license and proof of insurance
* Must have reliable and accessible auto vehicle.
* Must pass necessary fingerprinting, Level II background checks and employment eligibility verification through the U. S. Department of Homeland Security’s E-Verify system, https://e-verify.uscis.gov/emp.
