Über

Professional Case Management is seeking a compassionate, reliable Certified Nursing Assistant to provide high-quality in-home care for clients with complex medical needs, including former energy workers and their families. As a CNA in our Healthcare & Medical Services department, you will deliver personalized, one-on-one support that promotes each client’s comfort, dignity, and independence in the familiar surroundings of their own home.
In this role, you will assist clients with activities of daily living such as bathing, grooming, dressing, toileting, and safe mobility, while carefully following individual care plans. You will obtain and record vital signs, observe and document changes in condition, and promptly communicate concerns to the supervising nurse or care team. Under appropriate direction, you may assist with basic medical tasks such as simple wound care support, range-of-motion exercises, and proper use of medical equipment. You will also help with light housekeeping, meal preparation according to dietary needs, and support with errands or appointments as needed to maintain a safe, clean, and comfortable environment.
You will collaborate closely with nurses, therapists, and other healthcare professionals to ensure coordinated, high-quality care. Strong communication skills are essential, as you will interact regularly with clients and their families, providing clear updates and compassionate reassurance. You are expected to adhere to all infection control, safety, and privacy standards, maintain accurate and timely documentation, and follow agency policies and procedures at all times.
