Occupational Therapist

CenterWell Home Health is seeking an Occupational Therapist to provide compassionate, in-home therapeutic care to adult and senior patients. In this role, you will perform comprehensive evaluations, create individualized treatment plans, and deliver evidence-based interventions that promote safety, independence, and quality of life at home. You will collaborate closely with an interdisciplinary team, educate patients and caregivers, and document care in our EHR system.
