Job Description Title: Nursing Professional - Long Term Care
Schedule: - Shift: 08:00 - 16:30 - Days: Flexible, Monday through Friday with weekends as needed or Tuesday through Saturday.

Requirements: - License: Valid nursing license - Certifications: Relevant certifications in long-term care - Experience: Previous experience in skilled nursing or long-term care settings
Roles & Responsibilities: - Provide individualized patient care based on comprehensive assessments. - Facilitate group or concurrent therapy sessions as appropriate. - Collaborate with interdisciplinary teams to develop and implement care plans. - Monitor and document patient progress and response to treatment. - Ensure compliance with health and safety regulations. - Maintain accurate patient records and documentation.
