Collaborate with community partners and internal teams to deliver seamless, wraparound care
Track progress, maintain accurate client records (95%+ accuracy), and contribute to data-informed reporting
Celebrate success by sharing monthly client stories that highlight meaningful progress
Support long-term stability with follow-ups after clients exit the program
What You Bring
A strengths-based, trauma-informed approach and a heart for empowering people
Excellent communication, organization, and time-management skills

Comfort using tools like Salesforce, HMIS, and Tableau (or eagerness to learn)
A reliable vehicle, valid driver's license, and willingness to travel between sites
Flexibility for occasional evenings/weekends
Bilingual (Spanish preferred-but not required)
Preferred Background
BS in Social Work or a related field
4+ years of relevant experience (professional or volunteer)
Ability to obtain a Level One Fingerprint Clearance Card prior to hire
