Job Overview
We are seeking a dynamic and compassionate Patient Care Coordinator to join our healthcare team. In this vital role, you will serve as the primary point of contact for patients, ensuring a seamless and positive experience from appointment scheduling to follow-up care. Duties
Patient Care: Assist healthcare providers with patient examinations, treatments, and procedures. Take and record vital signs, medical histories, and patient information.
Documentation: Maintain accurate and up-to-date patient records, including care plans, progress notes, and any changes in patient status.
Crisis Intervention: Respond to patient crises and emergencies, coordinating necessary interventions and support services.
Quality Improvement: Participate in quality improvement initiatives to enhance patient care and outcomes.
Clinical Support: Prepare examination rooms, sterilize instruments, and ensure all necessary supplies are available.
Patient Communication: Educate patients about treatment plans, medications, and follow-up care. Provide emotional support and address patient concerns.
Administrative Duties: Schedule appointments, manage patient records, and handle billing and insurance documentation. Ensure accurate and timely entry of patient information into electronic health records (EHR) systems.
Compliance and Safety: Adhere to all healthcare regulations and safety protocols. Maintain a clean and safe working environment.
Qualifications:
Education: Licensed Practical Nurse
Licensure: Active and unrestricted license in the state of Louisiana (e.g., CMA)
Experience: 2-5 years of experience in care coordination, case management, or a related field.
Behavioral Health Experience a must
Skills:
Proficiency in taking vital signs and performing basic clinical procedures.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Proficiency in electronic health records (EHR) systems.
Knowledge of community resources and healthcare systems.
Preferred Qualifications:
Experience working with diverse populations.
Bilingual abilities are a plus.
Working Conditions:
Full-time position with a standard workweek.
Some on-call weekend hours may be required to accommodate patient needs.
Work may be conducted in various settings, including office, community, and telehealth environments.
