Summary
The Supportive Living Services Coordinator will carry out the administrative responsibilities for the
Supportive Living Services Program competently and consistently organize and supervise the SLS
program.
Education And/or Experience
Associate's degree (A. A.) or equivalent from two-year college or technical school; or six months to
one-year related experience and/or training; or equivalent combination of education and
experience. Prefer employee to have one year of supervisory experience.
Certificates, Licenses, Registrations
- Current, valid, California driver's license.
- Current CPR certification or the ability to obtain.
- Current First Aid certification or the ability to obtain.
- Current PRO-ACT certification or the ability to obtain.
Essential Duties and Job Responsibilities Include the Following:
- Responsible for interviewing prospective employees, and the Personal Attendant hiring
process
- Conduct new SLS employee orientation training.
- Schedule and conduct regularly scheduled PA training.
- Conduct regularly scheduled team meetings.
- Supervise PAs through weekly supervision meetings.
- Complete and maintain the work schedules for the Supportive Living Service Program.
- Write and conduct employee evaluations and competency assessments in a timely manner.
- Organize and supervise the direct provision of services to consumers.
- Perform consumer intakes and assessment.
- After assessment of consumer, the consumer's needs are implemented.
- Develop Individual Support Plans to delineate goals and objectives.
- Provide assistance in locating, securing and maintaining safe and suitable housing for the
consumer.
- Conduct home visits on a regular basis.
- Maintain all files of SLS consumers.
- Medication management for clients
- Any other job-related duties as assigned by the Program Manager, District Director, or
Regional Director.
Supervisory Responsibilities
- Will supervise PAs for those consumers on caseload.
- Surprise visits to the home to ensure staff are present, working and providing the services as
directed
- Provide monthly training to PA working with consumers
- Reconcile timecards with schedule for PAs and IHSS workers
- Will be available for after-hours on-call needs, as necessary
