About Us

Avivo's Ending Homelessness Division provides comprehensive services to individuals experiencing homelessness. Our programs connect people with shelter, housing, and long-term support using harm reduction, Housing First, and trauma-informed approaches. Avivo Village, one of our key programs, offers culturally responsive, low-barrier shelter and care to help residents achieve permanent housing stability.



Your Role

As a Site-Based Case Manager, you will support single adults with histories of chronic homelessness, many of whom live with substance use disorders. You will work closely with residents in our site-based program, Avivo Village. This role involves assertive engagement, harm reduction, and person-centered care to support residents in achieving housing stability and long-term goals.



Key Responsibilities Include
  • Build trusting relationships with clients through regular in-person engagement and creative outreach.
  • Maintain a caseload and meet monthly with all clients; complete creative contact attempts.
  • Conduct assessments and develop housing goal plans every six months.
  • Support clients in maintaining housing, addressing landlord issues, and resolving crises.
  • Provide eviction prevention and re-housing support, advocating for long-term stability.
  • Complete timely and accurate documentation in required systems (HMIS, Credible, etc.).
  • Collaborate with your team, assist with onboarding new staff, and attend required meetings.
  • Participate in Avivo's Undoing Racism and cultural responsiveness initiatives.
Qualifications

 What You Offer

  • High School Diploma or GED required; Associate degree or higher preferred.
  • One or more years of experience working with individuals experiencing homelessness.
  • Lived experience is valued and considered toward experience requirements.
  • Understanding of Housing First, Harm Reduction, and Trauma-Informed Care.
  • Strong communication, documentation, and advocacy skills.
  • Comfort working in unpredictable environments with a flexible, strengths-based mindset.
  • Valid driver's license, insurance, clean driving record, and reliable transportation.
