divh2Day Center CNA/h2pThe Day Center CNA (Certified Nurse Assistant) works at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care./pulliFollow the care plan and assist participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting./liliDocument care provided in accordance with established policies, guidelines, and applicable regulations./liliCarry out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews./liliAssist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed./liliMonitor participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and report pertinent information to clinical supervisor in a timely manner./liliCollaborate with multidisciplinary teams in care planning for participants./liliMonitor appetite and look for changes in functional ability and skin condition./liliMonitor/assist participants in a courteous manner with meals and helps with feeding, as necessary./liliReport any variance, accident, or unusual occurrence to the appropriate personnel./liliAssist with loading and unloading of participants from vehicles./liliAssist the transportation department with participants take-home supplies./liliServe as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities./liliMay rotate into In-Home Services as a Homecare CNA. as deemed necessary by the Center Manager./liliCreates and participates in group(s) and one-on-one activities in accordance with daily activity calendar when needed./liliAttend in-service and continuing education programs as scheduled and necessary./liliOther duties as assigned./li/ulpRequired:/pulliActive and valid state-issued Certified Nursing Assistant (CNA) license/liliCurrent CPR certification or the ability to obtain CPR certification.
