Primary Care Physician

Oak Street Health is seeking a compassionate and mission-driven Primary Care Physician to join our Healthcare & Medical Services team, providing comprehensive primary care to adults on Medicare in underserved communities. As a physician in our network of innovative, value-based primary care centers, you will deliver high-quality, preventative, and patient-centered care that improves health outcomes and access for vulnerable populations. In this role, you will manage a dedicated panel of adult and senior patients, focusing on building long-term, trusting relationships and understanding each patient’s medical, social, and behavioral health needs. You will provide routine primary care, chronic disease management, acute care for urgent issues, preventive screenings, and care coordination, leveraging a team-based model that includes nurses, medical assistants, care managers, social workers, and other allied health professionals. You will conduct in-person visits and may also use telehealth when appropriate to increase access and continuity of care. Responsibilities include performing comprehensive history and physical exams, developing and adjusting evidence-based care plans, ordering and interpreting diagnostic tests, prescribing medications, and coordinating referrals to specialists and community resources. You will use an electronic health record to document patient encounters, track outcomes, and support population health initiatives. Working within Oak Street Health’s patient-centered, inclusive, and collaborative culture, you will actively participate in multidisciplinary team huddles, case reviews, and quality improvement initiatives aimed at reducing hospitalizations, closing care gaps, and enhancing the overall patient experience. You will be encouraged to innovate in care delivery, contribute to best practices, and mentor or support other clinical staff where appropriate. The ideal candidate is a board-certified or board-eligible Internal Medicine or Family Medicine physician with experience or strong interest in caring for complex, geriatric, and Medicare populations. You are comfortable managing multiple chronic conditions, addressing social determinants of health, and collaborating across disciplines. Strong communication skills, cultural humility, and a passion for serving underserved communities are essential.
