Primary Care Physician
to serve as a
Clinical Team Lead
in San Bernardino, California. This role is ideal for a provider who is passionate about whole-person care, enjoys working in an integrated team, and wants to make a meaningful impact on medically complex older adults in a community-based setting. This organization offers a highly coordinated, patient-centered model that integrates medical, social, and behavioral services for individuals who require more support than the typical primary care environment provides. As the Clinical Team Lead, you ll guide a small care team while managing a manageable panel of patients through relationship-driven, longitudinal care. Key Responsibilities
Lead and support a cross-functional care team delivering comprehensive primary care
Provide both clinic-based and occasional in-home visits to enrolled patients
Emphasize continuity and prevention rather than episodic or volume-driven care
Collaborate closely with behavioral health, social work, therapy, and nursing professionals
Participate in a light call rotation (phone-only, a few nights or weekends per month)
Compensation and Benefits
Base salary:
$315,000 to $380,000, depending on experience
Quarterly performance incentives

$10,000

Ideal Candidate Profile
Board Certified or Board Eligible in
Internal Medicine, Family Medicine, Geriatrics, or Emergency Medicine
Experience or strong interest in working with aging and medically complex populations

Eligible for Medicare and Medicaid provider enrollment in California
