This role is responsible for supervising clinical staff, ensuring care is delivered safely, effectively, and in compliance with agency, state, federal, and accreditation standards.The Clinical Supervisor serves as a key operational and clinical resource supporting patient outcomes, staff performance, care coordination, and day-to-day workflow management across the Home Health and mobile care continuum.Key ResponsibilitiesClinical Leadership & Home Health OperationsProvide clinical supervision and coordination of Home Health patient care servicesEnsure care delivery aligns with physician orders, individualized care plans, and agency policiesSupervise and support field clinicians to maintain competency, compliance, and quality patient careProvide coaching, mentorship, and performance feedback to clinical staffServe as a clinical resource for escalated patient, caregiver, and staff concernsCare Coordination & DocumentationUtilize electronic medical records (EMR) to coordinate and monitor patient care servicesReview clinical documentation, including OASIS and visit notes, for accuracy, completeness, and timelinessSupport coordination of referrals, admissions, transfers, and dischargesCollaborate with interdisciplinary teams to ensure seamless patient transitions and continuity of careCompliance & Quality AssuranceHelp ensure compliance with Medicare Conditions of Participation and regulatory requirementsParticipate in quality improvement initiatives, audits, and corrective action plansPromote adherence to HIPAA, agency standards, and patient safety protocolsSupport operational excellence and continuous improvement within Home Health servicesQualificationsRequiredCurrent Registered Nurse (RN) license in the practicing stateCurrent CPR certificationMinimum of 2 years of Home Health experienceStrong knowledge of Home Health regulations, documentation, and patient care coordinationPreferred2+ years of clinical leadership, supervisory, or management experienceExperience supervising field-based or mobile care cliniciansKnowledge of OASIS documentation and Medicare complianceIdeal CandidateThe ideal Clinical Supervisor is:A collaborative and compassionate healthcare leaderExperienced in Home Health and mobile patient careOrganized, detail-oriented, and solutions-drivenSkilled in communication, team leadership, and conflict resolutionPassionate about delivering high-quality in-home patient careKnowledge, Skills & AbilitiesHome Health clinical operations and workflow managementLeadership, staff supervision, and employee developmentEMR documentation and OASIS reviewHIPAA compliance and patient confidentialityExcellent communication and interpersonal skillsStrong organizational, decision-making, and problem-solving abilitiesAbility to foster a positive, respectful, and team-oriented cultureWork EnvironmentThis role requires a combination of office-based and clinical coordination responsibilities, including sitting, standing, walking, typing, reviewing documentation, and communicating with clinicians, patients, and healthcare partners.
