- Primary Care Full time, contract position Location: Dublin I am seeking a CORU reg, Social Care Worker for a Primary Care Team in Dublin.
The SCW will support varying minority groups across all age ranges.
These include individuals, their families, children's parents, older persons and vulnerable adults with disabilities.
The SCW assists and empowers people to live safely and independently in the community, under the guidance and direction of the Primary Care Social Work Team.
The role is person-centred, rights-based and focused on promoting wellbeing, safety, inclusion and advocacy for service users and their families.
The SCW will provide direct practical, social and emotional support to Social Work Clients in the community, implementing care and support plans developed by the Primary Care Social Work Team. xyswxtq
They would promote independence, safeguard vulnerable adults and contribute to early intervention and prevention by supporting people to manage difficulties before they escalate into crisis.
