Hospice PRN CNA

Concho Hearts Hospice is seeking a compassionate prn hospice cna / hospice aide to support patients and families. what you'll do:

• provide compassionate personal care to hospice patients in their homes or care facilities.
• assist with daily living activities while ensuring patient comfort and dignity.
• observe and report patient condition changes to the nursing team.
• document care and communicate with the interdisciplinary hospice team.
• build meaningful relationships with patients and their families.
