MINIMUM QUALIFICATIONS:
Exemplifies characteristics of The BAYADA Way: compassion, excellence, and reliability
Demonstrated record of strong interpersonal skills
Demonstrated track record of successfully taking on more responsibility with positive results and ambition to grow past current position
Prior health care, and/or home or social service experience preferred
Competence in basic PC skills required to perform job functions
PRIMARY RESPONSIBILITIES:
Demonstrate and communicate the core values of BAYADA and The BAYADA Way
Conduct supervisory visits of non-medical staff in the clients’ homes and in the community
Ability to establish positive supportive and trusting relationships with and among clients, field staff and colleagues and work collaboratively and effectively with an interdisciplinary management team
Ensure field staff have completed all required competencies and documentation to provide required Homemaker and/or personal care assistance services
Visit clients in their home as required to:-Obtain feedback and ensure client and employee satisfaction-Ensure care plan is carried out as written
Report any service deviations or care plan requests from the client to the clinical manager and/ or client service manager- Identify and assist our client’s in resolving unmet social needs
Document all pertinent situations and interactions with clients, families, physicians, case managers and other care providers, to ensure adequate coordination and continuity of service
Develop community relationships and participate in outreach activities
