Education:
* Bachelor of Science degree in Speech Language Pathology required.
Essential Functions
* Screens and evaluates patients/residents and develops appropriate care plan.
* Under a physician's direction, provides direct patient/resident treatment based on established plan of care.
* Plans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and/or swallowing or cognitive problems.
* Administers, scores, and interprets specialized hearing and speech tests.
* Instructs residents/patients, families, and caregivers to monitor speech and provide ways to practice new skills.
* Completes required forms and clinical records in accordance with company policy and state and/or federal regulations.
* Documents in patient records in an accurate and timely manner.
* Devises special tools and treatments for the specific needs of patients.
* Provides comprehensive and compassionate therapy services in a sub-acute/skilled nursing facility setting.
* Works collaboratively with other rehab therapists and nursing staff in a supportive team-working environment.
* Maintains resident/patient privacy pursuant to HIPAA.
* Ensures punctuality and regular attendance for assigned shifts.
Other Functions
* Develops treatment plans, including short and long-term goals and methods to achieve identified goals.
* Communicates the resident’s/patient’s speech therapy program to the family and/or guardian.
* Keeps written documentation including evaluations, daily notes and weekly summaries.
Performs other duties as assigned.
