Profile Requirements
Resume with psych experience and age groups listed Ex. Psych CNA/BHT - Facility - (AGE GROUP/POPULATION) - DATES RTO cannot exceed 7 days (RTO will not be approved 6/22-8/2) Psych skills checklist (within the last year) 2 supervisory references from last 2 years of employment Certifications - certifications don't need to be active at time of submission but must be willing to obtain. Please include CPI if they have it. 8 hours of NBO will be added to the COA if they do not have an active CPI since they will complete it onsite. License - confirm they are willing and eligible to obtain (please include this in the profile) Driver's license or candidates address must be included in the submission packet (35 miles or less will result in rate decrease)
