About the Role: As an Evening Shift LPN at Silver Health Care Center, you will be responsible for delivering quality care to our residents, including: Administering medications and treatments as prescribed by physicians
Monitoring and reporting changes in residents' conditions
Providing personal care and assistance with daily living activities
Developing and implementing individualized care plans
Collaborating with interdisciplinary team members to ensure comprehensive care
Maintaining accurate and detailed records
Requirements: Current LPN license in the state of New Jersey
High school diploma or equivalent required
Ability to work effectively in a fast-paced, dynamic environment
Excellent communication and interpersonal skills
Strong attention to detail and organizational skills
