Provide comprehensive primary care for adult and senior patients Manage chronic conditions and preventive health services Develop long-term patient relationships Collaborate with interdisciplinary care teams Practice within established clinical guidelines and quality measures Participate in population health initiatives Qualifications
Board Certified or Board Eligible in Internal Medicine or Family Medicine Eligible for or active North Carolina medical license Comfortable caring for older adults with multiple chronic conditions Interest in preventive, value-based care Compensation & Benefits
