Internal Medicine Physician

Renuven Health Partners is a physician-led medical group dedicated to providing exceptional, patient-centered care for older adults in post-acute and long-term care settings . We are seeking a PRN Physician (MD/DO) to provide coverage in three skilled nursing and long-term care facilities in the area of Murfreesboro, Cookeville, McMinnville, and Columbia, TN .

Responsibilities

• Provide primary and transitional care services to patients in skilled nursing and long-term care facilities
• Perform patient assessments, diagnoses, and treatment planning
• Manage acute and chronic medical conditions
• Complete required documentation and Medicare recertification visits
• Collaborate with facility staff and interdisciplinary care teams
• Educate patients and families regarding treatment plans and goals of care

Qualifications

• MD or DO with active medical licenses in Tennessee
• Experience in post-acute and long-term care preferred
• Hospitalist, inpatient, or primary care experience welcomed
• Strong clinical judgment and commitment to quality patient care
