Job Summary:

We are seeking a compassionate and skilled LPN Licensed Practical Nurse to join our dedicated team of healthcare professionals at Silver Healthcare in Cherry Hill Township, New Jersey. As a valued member of our team, you will provide high-quality patient care and support our mission to deliver exceptional care to our residents.

Responsibilities:

• Provide direct patient care to residents in our long-term skilled nursing and short-term rehabilitation programs

• Administer medications, vaccinations, and treatments as prescribed by physicians

• Monitor vital signs and report any changes to the nursing team

• Develop and implement individualized care plans to meet the unique needs of each resident

• Communicate effectively with residents, families, and other healthcare professionals to ensure seamless care transitions

• Participate in interdisciplinary team meetings to discuss resident progress and develop care strategies

• Maintains accurate and up-to-date records of patient care and treatment

Qualifications:

• Current LPN license in the state of New Jersey

• Basic Life Support (BLS) certification

• Ability to lift, transfer, and reposition patients as needed

• Excellent communication and interpersonal skills
