Practices within the VA Nurse Practice Act and LPN scope of practice. Ensures compliance with HIPAA regulations to maintain patient confidentiality. Demonstrates respect for patient dignity with no reported breaches or complaints from patients or fellow care providers. Performs data collection and nursing interventions safely following physician orders. Continuously reassesses patients and promptly reports changes in condition to the RN. Provides patient teaching and follow-up care as appropriate. Supports patients in Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs). Involves family members in care and patient education. Maintains a clean, safe, and supportive environment. Prepares light meals as needed. Coordinates patient care with other healthcare providers and interdisciplinary team (IDT) members ensuring accurate handoff communication. Consistently follows CDC hand hygiene guidelines and utilizes personal protective equipment (PPE) appropriately. Demonstrates knowledge of safe patient handling techniques and equipment. Identifies fall risks and implements preventive measures. Uses patient two-identifier verification consistently. Appropriately labels specimens at the bedside. Ensures 100% compliance with the Five Rights of Medication Administration. Demonstrates knowledge of all prescribed medications including high-risk drugs. Maintains secure medication storage and handling. Assesses pain relief after medication administration and promptly reports unrelieved pain to the RN. Organizes priorities effectively and demonstrates knowledge of equipment usage and troubleshooting. Communicates patient complaints or concerns to the Clinical Manager and RN care manager. Accurately documents care including patient assessments, interventions, and outcomes in the Electronic Medical Record (EMR). Implements care according to established standards, policies, and patient age/developmental levels. Uses SBAR (Situation Background Assessment Recommendation) for patient handoff communication. Adheres to isolation policies and infection control guidelines. Maintains nursing bags and medical supplies according to guidelines. Performs phlebotomy, point-of-care testing, specimen collection, wound care, and other procedures accurately. Obtains vital signs, weights, intake/output, and point-of-care testing measurements correctly. Assists with patient assessment under RN guidance identifying physical, psychosocial, and environmental needs. Evaluates the patient environment for safety risks and hazards. Provides effective communication with patients, families, team members, and other healthcare professionals. Verifies documentation, paperwork, and laboratory requirements. Confirms procedures with patients before performing them. Completes thorough medication reconciliation when needed. Administers medications as ordered within LPN scope and follows disposal protocols. Demonstrates ability to perform medication calculations for mixtures and solutions. Actively participates in quality improvement projects including unit council and QAPI meetings. Prepares for accreditation by maintaining environmental standards and policy compliance. Demonstrates professional behavior in challenging situations and maintains positive interactions with the team. Assumes responsibility for ongoing education and competency development. Mentors student nurses and new team members as needed. Promotes the hospice philosophy, policies, and procedures for quality end-of-life care. Demonstrates commitment to minimizing waste and reducing costs. Maintains reliable attendance and flexibility with assignments. Attends at least 75% of staff meetings and participates in in-service training as requested. Demonstrates professional growth and competency through annual education and self-directed learning.
Required Qualifications:
Education: Graduate of an Accredited School of Vocational/Practical Nursing Licensure/Certification: Current Commonwealth of Virginia license as a Licensed Vocational/Practical Nurse or a multi-state license CPR- Basic Life Support Healthcare Provider certification (BLS HCP) Experience: None Drivers License: Yes To drive an Augusta Health Vehicle must also be 21 years old and Motor Vehicle (MVR) must be approved by Risk Management.

Education: None Licensure/Certification: None Experience: Two-years' experience as a Licensed Vocational/Practical Nurse in a clinical care setting Hospice experience Prior experience in a hospital healthcare system or related service-oriented environment Familiarity with Augusta Health’s systems, workflows, or organizational culture is a plus
