Case Managers will help youth and families in our community live healthier and more fulfilling lives through identifying and supporting their needs and goals. 12 hour shifts. Case Managers specialize in: Managing the care provided to youth in crisis Helping youth and their families gain access to the services they need to meet their goals External collaboration with community partners Provide crisis services in line with CCBHC standards Primary duties for case managers include: Collaborating with a multi-disciplinary team to assist in stabilization, de-escalation, and care coordination. Developing and monitoring a treatment plan that supports the person’s goals Helping obtain services needed to support goals, such as food or housing assistance, medical care, Social Security benefits, transportation, skills training, etc. Advocating and serving as a link between healthcare providers, employers, community agencies and other Oaklawn services Documenting all services into the electronic record Providing client care based on level of need Interacting frequently with the person’s treatment team including outpatient therapists, physicians, other treatment providers involved, community agencies, referral sources and other services. 12 hour shifts - days Minimum Hiring Requirements for case managers: BA/BS in social service or a human services-related field required Two years’ experience in a social service setting preferred Must have a valid driver’s license and proof of insurance
