She Recruits is seeking an experienced Manager of Case Management to oversee day-to-day operations of the case management program for a leading acute care hospital. This role will support high-quality, patient-centered care by ensuring effective coordination, discharge planning, patient throughput, and resource utilization across assigned units. Candidates may be Registered Nurses or Licensed Clinical Social Workers with strong leadership experience in an acute care setting.

Position Overview

The Manager of Case Management reports to the Director of Case Management and is responsible for supervising professional and support staff, ensuring consistent application of case management best practices, and supporting optimal clinical outcomes for patients. This leader functions as a subject-matter expert, resource, and advisor while maintaining operational oversight of workflows, performance, documentation, and regulatory compliance.

Key Responsibilities

  • Coordinate and oversee all case management operations for assigned departments
  • Lead, coach, and supervise the case management team, ensuring adherence to policies, documentation standards, and regulatory requirements
  • Promote high-quality patient-centered care through effective care coordination and discharge planning
  • Support patient throughput initiatives and monitor resource utilization
  • Serve as a subject-matter expert for clinical and social work case management practices
  • Review staff assessments, documentation, and interactions for quality and completeness
  • Collaborate with interdisciplinary teams to support smooth, timely transitions of care
  • Identify opportunities for process improvement and implement corrective actions as needed

Required Qualifications

  • Bachelor's degree in Nursing OR Master's degree in Social Work (LCSW required or LMSW with eligibility per state regulations)
  • 3+ years of clinical acute-care nursing OR hospital social work experience
  • 2+ years of supervisory or leadership experience in an acute care environment
  • Current state licensure as RN or LCSW/LMSW
  • InterQual experience preferred
  • Case Management certification or InterQual/IQCI certification preferred
