Performs duties and responsibilities in a manner consistent with the Mercy mission, values, and service standards.
The Care Management model provides effective transition planning and length of stay oversight to maintain patient experience, safety, and quality of care utilizing performance metrics and adoption of best practices. Position Details: Required Education: •
Graduate of an accredited School of Nursing, required •
Bachelor's Degree of Science in Nursing, minimum required Experience: •
2-3 years acute care hospital setting, preferred. •
Care Management or Utilization Management experience, preferred Licensure: •
Current License in the state of employment, required Certifications: •
BLS (CPR) at hire date, required, or within 90 days of hire •
Certification in Case Management, Preferred
