About the Job: As a Licensed Nursing Assistant, you'll play a key role in delivering compassionate, high-quality personal care to patients in their homes. You'll work under the direction of an RN and collaborate closely with the care team to support patient comfort, safety, and well-being. This role is hands-on, relationship-driven, and essential to helping patients remain safely and comfortably where they want to be-at home. What You'll Do:
Provide personal care services including bathing, dressing, grooming, toileting, and safe transfers. Support household needs such as light housekeeping, laundry, and meal preparation (after completing personal care tasks). Use mechanical devices and proper body mechanics to ensure safe mobility assistance. Observe and report changes in physical or mental condition, intake/output, and overall patient status. Cue patients for medications as directed in the care plan. Maintain a clean, safe, and healthy environment following aseptic and safety standards. Communicate professionally with patients, families, and the care team. Complete timely and accurate documentation. Uphold professional boundaries and represent the agency with excellent customer service. Travel within the agency's service area to provide care. Who You Are:
A Licensed Nursing Assistant in good standing with the New Hampshire Board of Nursing. CPR certified, with a satisfactory background check and motor vehicle record. Reliable, compassionate, and committed to delivering exceptional patient-centered care. Comfortable working independently in the home setting while collaborating with a supportive clinical team. Preferred: experience in home care and/or caring for patients with dementia, Alzheimer's disease, COPD, cancer, or post-surgical needs; ability to perform PT/INR checks. Why You'll Love Us:
