Support residents with daily living activities that promote comfort, dignity, mobility, and independence. Observe resident conditions and communicate changes or concerns to licensed nursing staff Assist with specimen collection, resident preparation for activities, and routine clinical support tasks Help maintain a safe, comfortable, and supportive care environment focused on dignity and consistency Collaborate with interdisciplinary healthcare teams to support individualized resident care plans Accurately document care activities and assigned resident information as directed QualificationsRequirements
Graduate of a Medicaid-approved Nursing Assistant program with certificate of completion Active Kentucky State Registered Nurse Aide certification required Basic Life Support (BLS) High School Diploma or GED required Ability to meet facility onboarding and screening requirements Ability to support resident mobility, positioning, and daily care activities in a long-term care environment Preferred
Prior experience in long-term care, rehabilitation, behavioral health, or skilled nursing settings Experience working within collaborative interdisciplinary healthcare teams Previous experience supporting residents with memory care or mental health needs Additional Information All your information will be kept confidential according to EEO guidelines.
