Nurse Practitioner Role At Concho Hearts Hospice Looking for a flexible Nurse Practitioner role where you can focus on meaningful patient care instead of rushed appointments and high-volume schedules? At Concho Hearts Hospice, our Nurse Practitioners have the opportunity to build meaningful relationships with patients and families while providing compassionate end-of-life care. We are seeking a PRN Nurse Practitioner (NP) to support patients in the Beaumont, Port Arthur, and Orange areas. What You'll Do:
Complete face-to-face visits and clinical assessments for hospice patients. Evaluate hospice eligibility and recertification requirements in collaboration with the Medical Director. Provide symptom management and palliative support to patients and families. Complete history & physical assessments and clinical documentation. Collaborate with physicians, nurses, social workers, and the interdisciplinary hospice team. Communicate clinical findings and recommendations to support quality patient care.
What We're Looking For:
Active Texas Nurse Practitioner license. PECOS enrollment required for hospice certification requirements (or willingness to obtain). Minimum 1 year of NP experience required. Hospice, palliative care, family practice, home health, internal medicine, or related experience preferred. Strong assessment, communication, and critical thinking skills. Ability to work independently while collaborating with a team.
